New Virgin Care referral form consultation

Virgin Care are inviting partner agencies to comment on a new referral form for their service. Please take the time to complete the short survey via the link below and share this with your staff or other relevant networks. The survey closes on Monday 23rd April.
Wiltshire Children’s Community Services, provided by Virgin Care, is developing a single referral form for partners to use to refer children and young people into services.
The service has consulted internally with colleagues on the information that should be included on the form to ensure all information is captured when people make referrals.
